SERVICE SUMMARY:The ServiceLink Resource Center is a local office that provides information, referrals, and assistance to people with disabilities, older adults, and their families. The ServiceLink Resource Center Team will help consumers in making connections to services, assessment, and eligibility counseling for home and community based services, and nursing facility admission.
The center can be accessed by phone, email, walk-in, appointment, or letter.
Services are free and confidential.
